Upon arrival you will be greeted by one of our compassionate and knowledgeable receptionists…making your experience a pleasant one is their top priority. Please have your photo ID, your insurance card and prescription (order), if provided to you by your physician, available. Copayments and deductibles are required at the time of service. Self-pay patients will also be asked to make payment prior to service. You will be asked to complete a registration form and possibly an exam history form, depending on the procedure you are having. Our registration form and cat scan, MRI, and nuclear radiology exam history forms are available on-line, please feel free to print and complete prior to your visit. A Patient Bill of Rights and Responsibilities is available at our office upon request. Your safety and comfort are extremely important to us. All of our exams are performed by a licensed and certified technologist, under the supervision of our board-certified radiologist. A technologist will explain your exam and answer any questions you have prior to completing your test. Upon completion of your exam, please take a few minutes to complete a quick survey.
